Poinsettia Dedication Form

Please fill out this form to order Poinsettias (in memory, honor or thanksgiving of someone or an event) which will be placed in the sanctuary on December 21. The cost of each plant is $15.00. If you pay with a check, please make it out to St. Mark's Episcopal Church with "poinsettia" on the bottom line.
